*For patients enrolled in active periodontal maintenance due to prior treatment of periodontal (gum) disease.

Some Formalities

  • You have the right to cancel your membership at any time by submitting a written request however the plan is non-refundable.
  • Children may be members only as dependents of adult members.
  • Patients with periodontal disease are not eligible for simple cleaning under Skyview Smile Club Basic plan as we are required to treat the disease once diagnosed.
  • Payments need to be made at the beginning of the plan.
